Patient Navigator / Vaccination Coordinator / Employee Health Nurse

EAT’s Patient Navigator/Vaccination Coordinator/Employee Health Nurse coordinates care for patients and families coming into Anchorage to receive medical care. The incumbent serves to guide patients and families through the health care system; commitment and dedication to continuity and quality of care is imperative for success. In addition the incumbent oversees all aspects of EAT’s vaccination program and supports employee health initiatives in all locations.
Successful candidates will have proven understanding of the impact of family dynamics, cultural influences, psychosocial issues, and/or substance abuse issues on the health and well-being of the patient; he/she will know how to best develop strategies for the maximum benefit of the patient. Familiarity with community support services, public service/benefit programs, patient assistive technology, long-term restorative care services and specialty care is critical; likewise, innovative and creative problem solving, collaboration and patience, and determination are competencies that help define the ideal Patient Navigator role.

This is a full-time position located in Anchorage, with periodic travel opportunity to the region.

To compete for this position, candidates must possess:
 High School Diploma or GED.
 Two (2) years of related clinical experience in care coordination, patient navigation, case management services.
 Current, active, and unrestricted State of Alaska licensure as a Licensed Practical Nurse
 Experience in tribal health or rural health care is preferred.
 Experience in rural cross culture setting is preferred.
