POSITION SUMMARY

Performs professional nursing duties in the care of geriatric residents. Uses evidence based practice in providing direct care to the residents and supervision of paraprofessional staff. Reports directly to the Long Term Care Director of Nursing or designee.

 

PRINCIPAL DUTIES AND RESPONSIBILITIES

Initiates and utilizes the nursing process to meet the holistic needs of the patient and family while adhering to the professional standards of conduct, ethics, and practice.

  1. Observes the resident’s clinical condition: recognizes, identifies, and interprets serious and/or emergent situations while initiating appropriate nursing interventions.
  2. Integrates evidence based practices when developing plan of care and interventions.
  3. Develops and implements patient/family teaching programs based on mutually identified health care goals.
  4. Documents all observations, nursing interventions, therapeutic measures administered, and status of coordinated activities between nursing and other professional disciplines.
  5. Evaluates the residents’ plan of care and revises plan as needed.
  6. Safeguards the resident’s right to privacy by judiciously protecting information of a confidential manner.
  7. Understands and practices facility safety and emergency preparedness procedures.
  8. Assumes responsibility and accountability for individual nursing judgments and actions.
  9. Provides guidance and leadership to other personnel in the form of in‑services and orientation/preceptorship and participates in committee activities.
  10. Assumes charge nurse duties as assigned, including staffing, supervision and decision-making responsibilities.
  11. Participates in departmental and organizational performance improvement activities.
  12. Utilizes incident and medication error reporting protocols per policies.
  13. Facilitates resident care across multiple disciplines as a member if the interdisciplinary team.
  14. Supervises and mentors paraprofessional personnel in the delivery of care in accordance with an established plan of care.
  15. Monitors, evaluates and responds to CNA observations and documentation of residents’ behaviors, physical condition, and abilities.
  16. Recognizes respects and promotes the integration traditional native values and customs into daily care and routines of the resident.
  17. Other duties as assigned.

MINIMUM REQUIREMENTS

Associate or Diploma in Nursing from an Accredited College. Current licensure as a Registered Nurse.  Ability to lift up to 50 pounds and spend long periods of time standing or walking.  May be exposed to communicable diseases and hazardous materials.  Must be able to pass the annual core competencies assigned to the unit.  Must maintain continuing education requirements as mandated by state in which license is held.

Increases depending on experience, education and as determined by the hiring manager.
