Job Summary: Job incumbent provides respite, chore, personal care or a combination of services to families and Elders. Incumbent will work closely with the Program Coordinator to implement the client's plan of care.Brief Posting DescriptionEssential Functions

Representative Duties: Under direct supervision of the Home Care Program Coordinator, job incumbent can provide one or a combination of the following services:

1. Provides respite, chore, or personal care services to families who are caring for someone who is frail, has Alzheimer's or related dementia assigned clients, following instructions in the plan of care.

2. Provides services in a respectful and caring manner consistent with clients' values and lifestyle.

3. Works in a planned, organized and safe manner.

4. Prepare and maintain records, nursing notes, of client progress and services performed, reporting changes in client condition to the Program Coordinator.

5. Weekly phone calls with Program Coordinator to report concerns or problems with clients' health, and receive guidance and support from the Program Coordinator.

6. Provides assistance and supervision with activities of daily living (bathing, eating, dressing, toileting, light housekeeping, meal preparation and shopping.

7. Assist Elders who need assistance, stand-by-assistance, supervision or cues to perform activities of daily living (eating, dressing, bathing, toileting, transferring in/out of bed/chair and walking) and instrumental activities of daily living.

8. Keeps client's area and personal belongings clean, safe, and sanitary.

9. Complete monthly continuing education units.

10. Submits accurate timecards, signed by the Tribal Council, and accurate time sheets signed by the Client, and Nursing Notes, every two weeks.

11. Follows TCC and Home Care policies and procedures.

Other Responsibilities:

12. Performs other job duties as assigned.

Minimum Qualifications:

1. Must be 18 years of age.

2. High school diploma or equivalent.

3. Current CPR and First Aid training certificate.

4. Meet Qualifications for personal care, state statute 7 AAC 125.125.090.

5. Current Certified Alaska Nurse's Aide License and/or Personal Care Attendant certification preferred.

6. Must pass background check pursuant to federal Indian Child Protection and Family Violence Prevention Act requirements.

7. Must pass background check pursuant to Alaska DHSS and federal Indian Child Protection and Family Violence Prevention Act requirements.

Knowledge, Skills and Abilities:

1. Ability to maintain strict confidentiality.

2. Ability to read and write English.

3. Ability to work independently with minimal on-site supervision.

4. Must be able to pass Blood Borne Pathogens test.

Supervision: This position does not have any supervisory responsibilities.

Physical demands: Job incumbent must be able to lift and carry thirty pounds in support of equipment and supplies. Position requires frequent standing, walking and sitting. Position requires bending, stooping, and kneeling. Incumbent must be able to work effectively in a busy and sometimes stressful environment. Direct contact with blood or other potentially infectious materials to which universal precautions apply.

Summation: Incumbent must have the ability to provide services in a respectful and caring manner consistent with client's values and lifestyle and performs assigned tasks in a safe manner. Must be able to work independently with minimal on-site supervision. Home Care Providers help patients with some of the most basic and personal tasks: to be successful, it is essential to treat patients with empathy, dignity, and respect.
