General Functions:
The Nurse Case Manager supports individuals on their recovery journey through evidence-based care, service coordination, and close collaboration with our psychiatric provider to ensure comprehensive medical and behavioral health support in both residential and outpatient settings. This position directly supports CITC's mission to develop opportunities that fulfill our people's endless potential by delivering strength-based, person-centered, and trauma-informed care. Working in partnership with members of the participants' treatment teams and external providers, the Nurse Case Manager plays a critical role in coordinating holistic healthcare services that promote wellness and recovery.

- Conduct comprehensive nursing assessments for residents in treatment, as appropriate.
- Develop and implement individualized care plans addressing medical, behavioral, and recovery needs in collaboration with participants and primary providers.
- Monitor withdrawal management and medication-assisted treatment (MAT) protocols in collaboration with the program Nurse Practitioner.
- Work closely with the Nurse Practitioner to support the provision of quality clinical care.
- Ensure correct medications are entered in the eMAR and maintain accurate medication lists.
- Obtain medical and psychiatric histories and collateral documentation.
- Track lab results and coordinate timely review with the Nurse Practitioner.
- Prepare and send referral forms to specialists as directed.
- Coordinate with physician and specialist offices to obtain visit notes and collateral for continuity of care.
- Educate residents on harm reduction, relapse prevention, and health maintenance strategies.
- Ensure compliance with state and federal regulations, including HIPAA and 42 CFR Part 2.
- Document all interventions and maintain accurate electronic health records.
- Participate in interdisciplinary team meetings and quality improvement initiatives.
- Collaborate effectively with members of multidisciplinary treatment teams.
- Maintain confidentiality according to all state and federal regulations, policies, and procedures.
- Strong communication skills, including active listening, clear expression, and the ability to document clinical information accurately.

- Continued employment is contingent upon completion of a satisfactory state and federal fingerprint criminal background check.
- Graduate of an accredited school of nursing.
- Licensed as a Registered Nurse (RN) in the state of Alaska at the time of hire.
- Minimum two (2) years of nursing experience in substance use treatment, behavioral health, or residential care OR Licensed Practical Nurse in the state of Alaska at time of hire with minimum four (4) years of nursing experience in substance use treatment, behavioral health, or residential care.
- Knowledge of addiction medicine, withdrawal management, and MAT protocols.
- Familiarity with Electronic Health Records (EHR).
- Familiarity with state and federal regulations (e.g., SAMHSA, Medicaid, HIPAA, 42 CFR Part 2).
- CPR certified or ability to be certified within 30 days of hire.
- Experience providing direct care in both residential and outpatient behavioral health and/or substance use settings.
- Certification in Addiction Nursing (CARN) or Case Management (CCM).
- Experience as a Registered Nurse Case Manager.
- Direct experience supporting the provision of addiction medicine, withdrawal management, and MAT protocols.
- Familiarity with ASAM criteria and integrated care models.
- May require prolonged periods of sitting, standing, and moving.
- Must be able to lift to 50 pounds occasionally, by utilizing proper lifting techniques and working in a safe manner.
- May involve the use of office equipment, tools, and other equipment as necessary. Possible bending, stooping, reaching, kneeling, and crouching.
